JOINT MASTER OF SOCIAL WORK PROGRAM
ADVANCED (CONCENTRATION) PRACTICUM APPLICATION
To complete your application packet please email copies of the following documents to:
Professor J. Kanode
Personal Resume.
Include Educational Objective, Work Experience, Volunteer/Previous Internship Experience, Special Skills/Competencies
(foreign language, computer proficiency), etc.
Proof of Professional Liability Insurance.
Due by August 1st.
This can be obtained one of two ways:
- NASW (must be a student member)
- Contact
UNCG Department of Social Work for the cost of the
Liability Insurance. The payment for the university
insurance is with a money order/bank check made out to UNCG Department of Social
Work. Payment is to be given to JMSW Field Co-Directors.
Proof of Automotive Liability Insurance.
One Letter of Recommendation from your foundation year faculty liaison.
You must complete this
online form which will be emailed directly to
Professor J. Kanode.
